Radiation release caused by mixture, not fire

The Accident Investigation Board Phase II Report determined the factors and causes that led to the February 14, 2014 radiological release at the Waste Isolation Pilot Plant last year. The AIB report concluded that the radiological release was caused by the improper mixing of organic materials, Swheat Scoop kitty litter, with nitrate salts in one drum that was processed at LANL and shipped to WIPP.
